just need to pay the $100 registration fee and
return the
Teenager Application
- If you have not held a
space, then return to us a
Teenager Application Packet along with the $100
registration fee

guardians please carefully read the Terms & Conditions,
Summer Teen Program Waiver and the Teenager Programs Checklist. Complete and sign all application/waiver
forms. Mail or fax your teen's completed application packet, including
the following: |

Form
- Summer Teen Program Waiver
- $100 registration fee - All
payments may be made with personal check, money order, VISA, MasterCard, American
Express and Discover. Full payment
is due 4 weeks prior to the start date. Applications and full payments received less than
four weeks prior to the start date will be subject
to an additional $35 late registration fee.
